One of our goals is to make sure that you get the most out of your visit to our office. Maximizing your experience requires preparation, patience and participation. In order to help you prepare for your visit, we have provided some helpful tips below.
You will need to bring the following items to your appointment:
- Your Insurance Card
- Photo ID
- Previous Medical Records: Include MRI/CT scan/bone densitometry or x-ray reports
- List of Your Medications: Include the dose and the times you take them
- List of Your Concerns: Include any issues or questions you want to discuss with your physician
- Your Payment: Please be prepared to pay your deductibles, co-pays, co-insurance and any other amounts due at the time of your visit
Registration Process
If you are a new patient, please arrive 30 minutes prior to your office visit to complete our in-office registration process.

On average, the time spent with each patient varies depending on the individual’s needs and complexity of their issues. Our doctors make every effort to stay on schedule and respect the time of every patient. Unfortunately, emergencies and unforeseen complications can arise and cause adjustments to scheduling.
